ABOUT YOUR FIRST CONSULTATION

After scheduling your initial consultation you will be given a HEALTH QUESTIONNAIRE which is to be filled out and returned at least two (2) days prior to your appointment. If you wish, our staff will email you with instructions on how to fill out the questionnaire online through a secure link.

 

You should allow one and one half hours (1.5 hrs.) for your initial Health Consultation. Here at Acacia Wellness Center we believe in giving full attention to each and every client. It is our desire to empower our clients in making well informed decisions regarding their health in a relaxed, unhurried atmosphere.

 

Please feel free to discuss any health issues. It is a good idea to write down specific questions and bring those to the consultation with you.
